Immunization & General Health Appraisal Requirements

As a licensed preschool with the State of Colorado, we follow the Colorado Department of Public Health and Environment requirements for immunizations. Please click here to learn more about the required vaccine schedule. Children must have up-to-date immunization records and a health appraisal form in order to attend school. Children must have a health appraisal per American Academy of Pediatrics guidelines/health care provider recommendations. If a child is on a delayed vaccination schedule, the schedule must be in accordance with the Advisory Committee on Immunization Practices recommended catch-up schedule, and we must have written documentation of the dates the vaccines will be administered. If a child has a medical, religious or personal exemption, we must receive a Certificate of Exemption form prior to the child's start of school and updated annually.
