Get a Contractor or Sub-Contractor Prequalification Application
Before you can bid on City of Louisville construction projects, you need to be prequalified. This page explains what to expect and what to prepare.
How to apply
1. Choose which application is right for you.
There are two application types. Choose the one that fits your role, or apply for both.
Application types
General Contractor
- You bid directly with the City
- You manage the whole project
- You may also apply as a sub-contractor
- Requires a certified audited financial statement
Sub-Contractor
- You work under a general contractor
- Bonding info & references still required
- Basic financial statement required
2. Gather your application materials
On your application form, you'll provide basic information about your business and experience. You'll also need to attach a set of supporting documents:
Both applications require:

Project list for the last 5 years. You'll fill out the project name, owner, engineer, contract amount, change orders, and completion date. Note whether you were the general contractor or a sub-contractor.

Resumes for key people in your organization

Financial statement showing your company's current financial position
General Contractor application also requires:

Current projects list. You'll fill out the name, owner, engineer, contract amount, percent complete, and expected finish date.

Staffing table showing how many people you have in accounting, construction management, estimating, engineering, quality control, labor relations, and safety

Certified audited financial statement. This must cover your most recent full fiscal year and be signed by a CPA.

Equipment list (what you own or can lease)
3. Have your completed application form notarized.
4. Return your form to Public Works.
